Confidentiality

Staff, volunteers and students who have access to organizational information use it only in the context of their work, and under conditions that guarantee its confidentiality. Clients who believe their confidentiality has been breached are encouraged to contact the agency.

Privacy Statement

SCWSS collects clients' personal and personal health information in order to provide services. Personal and personal health information is private. Unless sharing it is authorized by law, we cannot and will not give out any Client personal and personal health information without a Client's consent. A Client of SCWSS has the right to:

· See or have access to his/her personal and personal health information.

· Ask for and receive a copy of his/her health record.

· Ask us to correct or amend his/her records.

· Withdraw his/her consent to share personal health information at any time.

· Make a complaint to our Privacy Officer about access to his/her personal and personal health information, or about how it is collected, stored, used or disclosed to others.
