We provide on-site personal care and support services for seniors in designated Toronto Community Housing buildings.  Service is provided to eligible tenants who require frequent assistance with their Activities of Daily Living or need short-term help due to injury or illness.  Service is provided on a 24-hour basis by our dedicated Health Team.  Our Health Team is comprised of:

• Personal Support Workers
• Case Workers who provide care coordination
• Nurses (assessment and monitoring only)
• Social Work Counselors
• Community Development staff

Services may include:

• Personal care including bathing, dressing, toileting, hair washing
• Medication monitoring
• Meal preparation
• Safety checks
• Emotional support
• Light housekeeping and laundry
• Shopping
• Emergency response

This service is fully funded by the  LHINs.
