Reconnect (Community Services Unit) offers a range of services and programs which allow individuals to age at home with dignity and continuing independence.  Our services and programs address the needs of seniors and/or adults with disabilities at various points along the continuum of care from the most robust to frail adults and adults with Alzheimer's disease and/or dementia.   Please click on a link below to learn more about an individual service or program:
