Reconnect is committed to assisting seniors and adults with disabilities to live with independence and dignity in their homes.  There may be times when individuals and their families and caregivers are faced with issues, need someone to talk over their options with or discuss future plans.  In such cases, our Case Management Team is available to provide:

  • Counseling
  • Assistance in finding and coordinating appropriate services
  • Assistance with filling out forms
  • Education, training and support group referrals for family and caregivers
  • Advocacy
  • Crisis Intervention

Our friendly and professional Case Management Team can provide in-home and in-office assessments to plan and coordinate services to meet the needs of the individual.  Goals are set and expected outcomes are agreed upon and are documented in a care plan.  This process ensures the appropriate care is provided to assist seniors and adults with disabilities to age at home successfully.

Please contact our Intake Department at 416-787-2114 x 214 for our eligibility criteria to access services and programs.

We are able to provide our services in various languages including  Portuguese, Somali, Spanish, Urdu and others.

Both Case Management and Intake are provided free of charge.
